Ignite PVR recordings keep freezing and losing audio.

I have tried rebooting system, and it now won’t play recordings and gives a XRE03091 error.

I am not sure what you would like to see as the error counts have been cleared by a reboot 18 days ago. I do have some errors again, but only about 6000 or so.
