My sister moved in with me in Ottawa from Quebec. While she was told she could keep her number, she wasn't told that Quebec tax would be applied. She's now an Ontario resident. This doesn't make sense.

It can be perplexing to see Quebec taxes applied to your sister's phone number when she's now an Ontario resident. We have discussed a similar scenario in a different thread: PST Charged in Alberta on BC Area Code. In essence, taxes are calculated based on the province where the number is registered.
