I lost my phone and rogers is looking for Location Enabled Service Document. 

Where can i find this document?

Just to clarify, is it Rogers that is asking you for this info? Or did you put in a claim through Likewize for your premium device protection plan?

Also, are you referring to the "find my phone" service that you can set-up prior to losing your device?  What was the make/model of the lost device? We just need a bit more information to better understand your concern.
