I was having a similar issue with MMS on my Lumia 920 and while most of the settings above are correct, I found it only works if the MMSC APN is set to the following:
APN: ltemobile.apn
Username:
Password:
WAP gateway (URL): mmsproxy.rogers.com
WAP gateway port: 80
MMSC (URL): http://mms.rogers.com
MMSC port: 80
Maximum MMS size:
IP Type: IPv4

Thanks Friganka,
This solved my problems on my unlocked European HTC 8X running Win 8. The internet apn is set to ltemobile.apn, but I have the mms apn as:
media.com
username media
password mda01
and the rest as you suggest. First time it has worked in over a year on Rogers.

Nokia Lumia 1520 MMS APN settings
In case this is useful to anyone. Picture messaging wasn't working for me using the standard APN settings that Rogers support sent me via e-mail. Finally I got it to work with these settings
APN: ltemobile.apn
Username: <blank>
Password: <blank>
WAP gateway URL: 10.128.1.69
WAP gateway Port: 80
MMSC URL: http://mms.gprs.rogers.com/
MMSC Port: 80
Maximum MMS Size: 1000
IP Type: IPv4

Until today, I faced the same problems with my unlocked Telus 1020 and tech support couldn't help.
These Rogers APN/MMS settings are verified working as of June 1, 2015
On a Windows 8 phone go to Settings > cellular+SIM
Data connection - on
Data roaming options - roam
Highest conneciton speed - LTE
Select "Sim settings"
Select "edit internet APN"
> Save settings
Select "edit MMS APN"
> Save settings
Send yourself a test and everything should work now.

Called into rogers tech support, and they gave me the settings, but unfortanitly they don't work.
Anyone have some luck getting MS 950XL fully working.. Right now I have internet, text messaging but no picture sending/recieving.

Hello again @rcadams,
Thank you for that; I was actually a little intrigued with the way it was set up/entered in your phone.
However, I did some more digging since your original post and found something that might resolve your issue.
You can try to download the access point app from the Windows Store.
Access Point settings only work on phones that aren't locked to an operator.

I'm attempting this on th enew 950 XL as well. The access point app is no longer required. Under the SIM settings I am able to edit this directly in windows mobile 10. IT's interesting in that the standard APN settings get updated automatically (There is an option to pull these from the carrier) but there is nothing under the MMS settings at all.
