Contact names not displaying

I have an Iphone5 and whenever someone in my contacts list calls me their name does not appear ... rather it comes up as "unknown".  Any suggestions as to how to fix this issue?

I have an older plan for an iPhone 4S. Call display is included in my plan as part of the iPhone Value Pack + Vvmail option. Just checked the settings on my phone, which is on iOS 8.1.2, and there are no options to control call display.

 

Does your plan include the iPhone Value Pack or something similar?
