There are no rebate forms for that credit, it is applied to your next invoice:

I would call rogers if you didn't get it.

You can fill out your $50 mail-in rebate online directly in your account. You need to be logged in your account. In the section overview under your phone number (top middle of the page) you will see "personalize my phone" click on that. On this page it says get phone rebate. Click on that and then you've instantly filled out your rebate form and no need to mail it in. You will get an e-mail confirmation in about 48 hours. Hope this helps.
